We’re here for your whole family whenever you need us! We see all ages from newborns to adults!
We see newborns, toddlers, school-age children, teens, young adults, and adults for in-home visits! (Geriatric patients will be referred to our medical director for office visits)
Newborn visits ( Under 8 months old-sick and well visits) are $360 per visit.
Urgent care visits for children and adults are $260, and well visits for children and adults are $185.

No. Memberships are not required for any of our services.

Yes! We offer house call and telehealth memberships. Memberships start at just $75 for telehealth and $250 for full service (includes annual well checks, urgent care, and sick visits). All memberships include unlimited telehealth visits for covered members.

Yes! You can absolutely use our services while under the care of your primary pediatrician. We are happy to provide a detailed summary of the visit for your child's pediatrician.

Yes! We offer well visits, routine check-ups, prescription management, and school and sports physicals.

No travel fee if you are located within our service area. We are currently serving communities in St. Johns County and Duval County between Ponte Vedra and St. Augustine Beach and communities South of 295 and East of the St. Johns River, as well as the Beaches communities, including Jacksonville Beach. Please reach out to see if your neighborhood is in our service area! If outside our service area radius, a travel fee will be determined on a case by case basis.

Yes! We offer customized enrollment options for businesses and organizations interested in providing Care On Call Pediatrics & Family services to their employees. If you’d like to explore a group or corporate plan for your team, please contact us to discuss available options and create a tailored enrollment plan.

A valid credit or debit card is required on file at the time of booking. Your card may be charged before, during, or after your appointment for services rendered.
If a payment is declined, a late or insufficient funds fee may be applied to the outstanding balance.
Because your appointment time is reserved specifically for your family, cancellations made within 5 hours of the scheduled visit time are subject to a cancellation fee. If our provider arrives and the patient is not present, the full visit fee will be charged. Late arrivals may result in an additional late fee.
No refunds will be provided for cancellation fees, late fees, or credit card decline/insufficient funds fees.

Yes! We offer telehealth services across the entire state of Florida!
Our in-home visits are currently offered in St. Johns County, Duval County, and Flagler County between Ponte Vedra and St. Augustine and communities South of 295 and East of the St. Johns River, as well as Nocatee and the Beaches communities, including Jacksonville Beach up to Jacksonville Golf and Country Club and Atlantic Beach. Please reach out to see if your neighborhood is in our in-home visit service area!

We do not accept or process insurance claims at this time. We will provide a receipt of service which can be submitted to your insurance for an out-of-network claim. We fall under Telehealth and Urgent Care, which most insurance plans cover. We do accept HSA, FSA, and self-pay options. If you would like assistance with your claim we recommend using

Yes! We are open 7 days a week.

We strive to have a provider available for most holidays. If we are able to get a provider to you during a major holiday, an additional fee of $15-$50 will apply depending on service type and travel distance for the provider on call.

Yes! We can evaluate multiple family members during a single house call or telehealth visit. A reduced Additional Family Member fee applies for each sibling and/or adult family member seen during the same appointment.
This allows your family to receive convenient, coordinated care in one visit while saving time and minimizing disruptions to your day.

Our team consists of Board Certified Family Medicine and Pediatric Nurse Practitioners, Physicians, and Physician Assistants (PA-C). The providers you will be seeing with our practice are Larissa, DNP, FNP-BC and Jonathan, PA-C. Supervised by Dr. Elizabeth Marquez, M.D.

Nope! We understand that illness and injury don’t always happen during convenient business hours. We’re here for you when you need us, no additional charge for weekends, late nights, or early mornings.

Yes! We have providers on our team that are certified and licensed in family medicine and adult urgent care and emergency medicine and can treat adult family members!

We travel all across St. Johns County, and to many communities in Duval and Flagler County. We most frequently visit Ponte Vedra Beach, Nocatee, Sawgrass, Marsh Landing, Palm Valley, St. Augustine, St. Johns, Julington Creek, Beachwalk, Beacon Lake, St. Johns Golf and Country Club, Jacksonville Golf & Country Club, Jacksonville Beach, Durbin, Bartram, E-Town, Rivertown, World Golf Village, Silver Leaf, Shearwater, Trailmark. No. A single visit is considered a fee-for-service urgent care or well visit and does not establish ongoing care with Care On Call Pediatrics.
To establish ongoing care with our providers, enrollment in a Care On Call Pediatrics membership is required. While membership is not required to use our services, it may be a good option for families who plan to use house call visits as their primary source of ongoing pediatric care.

Your house call visit includes follow-up calls or text communication with a provider for up to 10 days after the visit for the same patient and the same condition evaluated during the original appointment. This allows us to monitor recovery and answer questions related to that specific concern.
Follow-up communication does not include evaluation or treatment of new symptoms, new conditions, or secondary illnesses. Any new or additional concerns will require a separate visit, either through an additional house call or a telehealth appointment.
If a second in-home visit is needed for the same illness, a follow-up house call may be scheduled at a reduced rate when booked within 30 days of the original visit.

If your baby required a NICU stay, experienced complications during delivery, or if there were significant complications during pregnancy, written medical clearance from a pediatrician is required before in-home newborn services can be provided, including the use of newborn package visits.

We offer a General Text Advice option for $45, which includes up to 10 minutes of provider time starting from when the provider responds. This service is intended for general, limited questions such as general medication dosing guidance or over-the-counter medication support.
General Text Advice does not include diagnosis, treatment plans, or patient-specific medical care and is not a substitute for a medical visit. Messages will be answered when a provider is available and responses are not guaranteed to be immediate.
If more detailed guidance is needed or if the conversation exceeds 10 minutes, you may upgrade to a telehealth visit for $75 for more comprehensive evaluation and care.
