Jonathan Salazar, MMS, PA-C
Owner & Founder of Care On Call Pediatrics, Physician Assistant
Jonathan Salazar is the founder and CEO of Care On Call Pediatrics. He is a highly experienced Physician Assistant with a Master in Medical Sciences from Nova Southeastern University. Jonathan has extensive expertise in pediatrics, trauma, emergency medicine, pediatric and adult urgent care, and pediatric orthopedics. With an additional background in child development and special education, and experience working with special needs children in schools and The Special Olympics, Jonathan is particularly skilled in techniques that comfort and calm children and alleviate anxiety, ensuring they feel safe and secure during their visits.
Jonathan brings a deeply personal and holistic approach to medicine—one that focuses on long-term wellness, prevention, and supporting the overall health of each child he treats. He partners closely with families to develop personalized care plans tailored to each child’s unique needs, health history, and developmental stage. These practices are not only a core part of Jonathan's healthcare approach but also a founding principle of Care On Call Pediatrics.
Jonathan lives in Nocatee with his family and is a devoted husband and father of two. His deep commitment to family, community, and compassionate care is reflected in every aspect of his practice.

Larissa Frauenheim, DNP, FNP-BC
Doctor of Nursing Practice, Pediatric and Family Nurse Practitioner
Larissa Frauenheim is a dedicated Family Nurse Practitioner with over 15 years of diverse nursing experience and a passion for delivering personalized, high-quality care. Board-certified by the American Nurses Credentialing Center, Larissa brings a compassionate, evidence-based approach to every patient interaction—empowering families to take charge of their children’s health and wellness.
Larissa earned her Doctor of Nursing Practice (DNP) from the University of North Florida and holds a Bachelor of Science in Nursing (BSN) from the University of Central Florida. Her clinical background spans pediatrics ICU, pediatric emergency, and adult emergency medicine, giving her a unique and comprehensive perspective on patient care.
Known for her commitment to preventative health, Larissa works closely with each family to create customized care plans that support long-term wellness. Her approach is rooted in building meaningful relationships, where children feel safe and parents feel heard and supported.
Outside of her clinical work, Larissa lives the healthy lifestyle she encourages in others. She enjoys spending quality time with her family, two young children and their beloved French Bulldog, Coco Chanel.

Dr. Sarah Adams, MD
Medical Director, Family Medicine Physician
Dr. Adams is a compassionate and dedicated Family Medicine Physician with a lifelong passion for patient care. Trained in pediatrics, internal medicine, and geriatric care, she provides comprehensive healthcare for individuals and families at every stage of life.
Her approach to medicine is deeply personal—she believes that healthcare should fit into her patients' lives, not disrupt them. She emphasizes wellness, preventative care, and lifestyle modifications to help her patients achieve lasting health. Dr. Adams is also a strong advocate for community involvement, frequently volunteering at local health fairs, offering sports physicals for students, and giving educational talks on wellness and disease prevention. Her commitment to patient-centered, accessible, and high-quality care is the foundation of her model of care. Dr. Adams lives in Nocatee with her husband, two young children, and their two pups.
